Quality of Life Elements

The Quality of Life Elements, discussed below, are an important part of an Individual's overall Quality of Life Plan.

Element We document progress and difficulties regarding...
Medical, Dental, and Other health-related issues  medical and dental appointments or recommendations, any health concerns, and changes in mood or physical condition
Communication   the expression of needs and wants, the status of speech therapy, and the use of a communication device or system
Personal Safety safety in one's home, community, and workplace, such as the ability to cross the street, adjust the water temperature of a bath, or ask for help when feeling uncertain
Mobility and Transportation physical supports needed to participate in one’s community and modifications needed to safeguard mobility, such as a walking device or a railing for climbing stairs
Human Awareness and Sexuality support, instruction, counseling, or professional consultation needed in relation to developing intimate relationships or expressing oneself sexually
Financial the management of one’s own money and financial responsibilities
Mentoring Support areas of difficulty currently receiving support
Other Lifestyle Issues any other areas that need support according to the person’s Quality of Life Assessment

See the form entitled Quality of Life Elements Assessment.  This document is used to assess an Individual’s possibilities and vulnerabilities in the eight areas described above.  In order for the Individual to be involved in his/her own quality of life, Caregivers support the Individual within these elements.
